just wondering if this is ever allowed. Specifically at T-Rex.

They may allow it, but I bet if they do, it won't include the Supersaurus Sampler or Colosso Beef Nachos for Two. Restaurants are very particular about not allowing one credit to pay for multiple credit items...even in this circumstance where you'd be using a full dining credit on it.

That’s fine. Wasn’t looking at those.

How does the dessert work though? Can 4 of us get the 4 person dessert?

Yes. It takes 4 people on the dining plan using their dessert credit together to order the Chocolate Extinction. Otherwise, you have to pay out of pocket for it.

Can one of these credits be a child chredit? Or does it have to be 4 adults?

Very unlikely you'll be able to order an app as an entree at T-Rex because it is a third party owned restaurant which is reimbursed by Disney according to the rules of the dining plan. They would only be reimbursed when an entree (from the entree section of the menu) is ordered so if a server allows it, you'll be getting major pixie dust. You would have a better chance of ordering an app in place of an entree at a Disney owned restaurant.
